Volunteer Resources

ALL volunteers (including those who have been serving SFP for some time) are now required to complete a background check to guarantee the safety of all children in the network, all guests and all volunteers.

All volunteers are also asked to sign a confidentiality agreement and complete a volunteer information sheet so that we can create a centralized database of volunteers, thereby making the job of our coordinators easier.

This sheet is to record contact information for volunteers as well as where they wish to help.

Reduced Fee Request Form

Complete this form and include it with your request. This will allow us to only pay $10.00, instead of $25.00 for the records check.

Criminal Records Release Authorization Form

Fill out both sections 1 and 2, and have the signature notarized.

Confidentiality Agreement

This is a statement of understanding about what information is confidential.
